This comprehensive session explores the full scope of professional case management practice, beginning with the evolution of the role and the defining principles of care coordination. Participants will examine the standards and philosophies established by leading organizations, gaining a clear understanding of how core competencies shape effective, patient-centered practice.
Building on this foundation, the program takes a deep dive into the essential components of case management, including the process, goals, and eight key activities that guide daily practice. Attendees will also explore the complex landscape of healthcare delivery and reimbursement, including care models, transitions of care, interdisciplinary collaboration, and the financial structures that influence patient access and outcomes.
A strong emphasis is placed on psychosocial considerations, equipping participants with strategies to address behavioral health needs, social determinants of health, and patient engagement across diverse populations. The session also highlights quality measurement and performance improvement initiatives, helping case managers understand how to evaluate outcomes, utilize data, and contribute to organizational excellence.
In addition, participants will gain practical insight into rehabilitation concepts, including return-to-work strategies, vocational considerations, and post-acute care planning. The program concludes with an overview of the ethical, legal, and regulatory frameworks that guide professional practice, ensuring participants are prepared to navigate complex situations with confidence and integrity.
Designed to be both practical and applicable, this session provides a well-rounded framework that supports case managers in delivering high-quality, coordinated care while strengthening their professional practice across the continuum.

Zack Anglin is the first quadruple amputee in the world to become a licensed commercial pilot and flight instructor! A three-time state-qualifying high school wrestler in his home state of Wisconsin, Zack’s limb differences have never defined him. When he announced his senior year of high school that he was going to learn to fly, many people were skeptical, but Zack was undaunted.
Zack moved to Tulsa, OK in 2018 after being accepted to Spartan College, but before he could begin flight school, he had to lobby the FAA to grant him a medical test to prove he could fly, a test he passed with flying colors.
From there, Zack began his pilot training, learning to adapt and manage a plane’s controls. His ingenuity and determination were boosted by the dedication of his Hanger Clinic prosthetist Scott Stromberg, who fits Zack’s prostheses and ensured that his devices fit perfectly in the cockpit.
In 2019, Zack earned his FAA license, and today he’s a flight instructor at Spartan, proving to his students that there is nothing they can’t overcome. Zack is grateful for his faith and family, including wife Martyana and their 3 daughters, who always lift him up. Keep going, Zack; keep redefining what’s possible!
